[Population Modeling] Sources of public medical data

Hi to all modelers working with public data,

Madhav was asking me yesterday about public sources of medical data.

I offered two well known pointers:
NHANES
AHRQ

Does anyone have other public sources they know about they wish to share
with the group.

It may help several of us sharing such knowledge of public repositories.
